Registration and Enrollment Information
Welcome to our registration and new to the district website. We're working to put the forms relating to enrolling and registering for the Keokuk Community School District at this site. Where applicable, forms may be printed, completed, and brought to registration.
Registration for all students attending the Keokuk Community Schools will be held in August 4-5, 2011 in the Keokuk Middle School gymnasium. Please use the gymnasium entrance at the rear of the school.

New Students to Keokuk Community School District
For anyone registering for the first time in the KCSD you must bring the following:
- Proof of birth date (e.g. birth certificate, hospital record of birth, baptismal record)
- Immunization records: diphtheria, pertussis, tetanus, polio, measles, mumps, and rubella.
- The name and address of your child's previous school.
All children must be immunized for these diseases before they may attend school. For more information, call (319) 524-1402.

Preschool
- Enrollment Application--Enrollment applications for 2012-2013 will be available in February/March 2012. At this time our voluntary preschool is full. If interested in being placed on a waiting list, please contact Donna Tracy, Hawthorne principal (319-524-3503 x3001).
Elementary (K-5)
- Dental Screening--Kindergarten only

Middle School (6-8)

- Physical Form - FEMALE - MALE
- School Supply Lists--MS students do not receive school supply lists as their classes vary. During the first few days of school, MS students will receive information from their classroom instructors regarding what supplies will be needed.
High School (9-12)
