Welcome to our enrollment and registration website. Below you will see information and forms related to enrolling and registering in the Keokuk Community School District.
Online registration for the 2020-2021 school year has begun! To register your student(s) please visit bit.ly/2CGkXxa to get started. We are excited to announce that this year students who are new to KCSD can register online. This includes families enrolling students in preschool and kindergarten. KCSD strongly encourages families to register their student(s) online for their own ease and to reduce the number of families at in-person registration.
Centralized registration will be held at the Keokuk Middle School Gymnasium on August 3rd, 2020 from 8 AM to 5 PM. Parking is available behind the school. At registration, you can setup your Parent Portal account, speak to the food service staff, sign up for bus transportation, pickup KHS Activity Passes, and schedules from guidance counselors, speak to building administrators, pay your fees, and visit booths set up by local interest groups.

New to our District?
For anyone registering a student that was not enrolled in our district on the last day of school, must provide the following:
- Proof of birth date (e.g. Birth certificate, hospital record of birth, baptismal record)
- Immunization records: diphtheria, pertussis, tetanus, polio, measles, mumps, and rubella. All children must be immunized for these diseases before they may attend school.
- Proof of Residency
- The name and address of your child’s previous school

Transportation
To determine bus eligibility and signup for bus service, please come to registration or contact the Bus Garage at 319-524-3690.
Statewide Voluntary Preschool Program
Students that are 4-years-old by September 15, 2020 and students that are 3-years-old through 4-years-old with an Individual Education Plan (IEP) may attend Torrence Preschool. Please contact Hawthorne Elementary & Torrence Preschool’s Principal at 319-524-3503.
Elementary (K-5)
Students in grades K-3 will attend Hawthorne Elementary. Students in grades 4-5 will attend George Washington Elementary.
- Dental Screening Form — Kindergarten only

Middle School (6-8)
- Physical & Concussion Forms – Required for athletics
High School (9-12)
- School Supply Lists — students do not receive school supply lists as their classes vary. During the first few days of school, students will receive information from their classroom instructors regarding what supplies will be needed.
- Dental Screening Form – required for all new students and ninth graders
